We are an Aging Life Care Manager™/Eldercare Consultant/Case Management company that is a resource for
assisting families, spouses, and adult children of aging parents with identifying community resources, providing referrals
to appropriate professionals, and ultimately reducing the overwhelming responsibilities of being a caregiver.
Our Healthcare Transition Partners’ team is dedicated to support and hold your hand during the entire process of
making decisions during a stressful time. Our approach is to work with the client, family, caregiver, and healthcare
providers collectively to develop an individualized care plan that will meet your immediate and future needs.
Our initial consultation can be over the phone or in-person. Our goal is to speak with clients/families to help identify their needs, in order to make recommendations for the best care options and solutions available. If you are local or remote from your parents, working with a care manager will bring additional support for the entire family.
We conduct an in-person or virtual assessment that covers a range of issues relevant to health and living situations, including everyday activities, nutritional status, safety, memory, depression, finances, and insurance. A Care Plan includes the results of the assessment, recommendations, and referrals for local care options.
The care plan is implemented during care coordination. The care manager will do all the planning and problem solving for the client, family, and caregiver. Coordinating care and collaborating with other community providers is what care managers do best!
We represent our clients’ best interests and do whatever it takes to secure the most favorable outcomes, for their care, comfort, and wellbeing. We provide family education, support, and coaching because being a caregiver is not an easy job.
Our Care Managers will arrange services through other companies when appropriate. We know the right people to get things done. This is especially important because it can save money in the long-term. We execute the care plan, act as liaisons between service companies, and often catch issues before they become problems.
We provide senior living placement options when individuals are unable to safely live in their homes after being discharged from a hospital or rehab facility. We consult with clients and their families to consider appropriate levels of care, additional support, lifestyle, location preferences, and more.

She has spent 16+ years as a licensed social worker and is a Certified Dementia Practitioner. Ms. Smith has extensive experience helping families navigate the healthcare system, and is passionate about educating and advocating for families in making informed decisions regarding healthcare options.
